It's unlikely the transmission when in P or N could cause a bad idle. Fix the idle problem first, then see what else needs to be done. Most often, idle problems are due to vacuum leaks -- or somebody fiddled with the idle curb speed screw and idle mixture screw.

 

 

Yeah if your truck is idling at 3000rpm it will lunge and stall. Fix the idle issue.
